The Code of Health and Disability Services Consumers’ Rights (the Code) establishes the rights of consumers, and the obligations and duties of providers to comply with the Code.
What is Active Families?
Criteria
-
Children aged 5–12 years
-
Reside in West Auckland, North Shore, Rodney, or Hibiscus Coast
-
Able to attend weekly activity sessions during school term
-
Currently inactive (less than 5 hours of physical activity per week)
-
Has a stable medical and/or mental health condition that could benefit from regular physical activity
-
Whānau/guardians are ready to make lifestyle changes and consents to referral
Referrals can be by self-referral, via your general practice team (doctor or practice nurse), or by any other health professional.
