FAQS
How do I get a Green Prescription/Rescript?
You will have to go to your GP or health professional and ask them for a referral, this is the process everytime you want to join the programme. We are currently not taking self referrals due to high demand.
How do i get started on the GRx programme?
You will need to have a consult with us, whether or not you have been on the programme before, to see if you still meet the requirements/qualify to be on the programme.
Once my Doctor has made the referral how long do I have to wait?
Currently our wait time is 4 – 5 weeks once we have received the referral. This is due to high demand for the programme.
When will I receive my Green Prescription card?
You will receive you Green Prescription card once you have had a consult with your Healthy Lifestyle advisor. If you have had a consult and are struggling to find your card in your emails, check your junk mail first.
Do I need to rebook into a consult for my activity update?
No – you do not. Please reach out to your Healthy Lifestyle Advisor and they will be in contact.
Do I need a wellbeing consult to get a programme for the gym?
No, after you have completed a Wellbeing Consult with your Healthy Lifestyle Advisor – they will have used this time with you to talk through your next steps for starting exercise. If one of these recommendations is you booking in with a Fitness Consultant at the Auckland Council LC of your choice to get a gym programme, this is something you arrange with the Leisure Centre, not as an additional booking with GRx.
I'm a health care professional, how do i refer patients to the programme?
You can refer via the Health Professional button to the left. This is password protected, call or email us for the password.
What services aren't included on the programme?
We are not a rehabilitation service, we do not offer one on one exercise training, we do not offer gym memeberships, we offer discounted access to Gyms and Lesiure Centres.
We are not a specialised service provider which includes:
- Mental health
- Counciling
- Injury rehab
- Complex or high needs
- Cronic conditions
We can provide generalised information for this conditions.
What is Active for Life – Green Prescription?
The Active for Life programme is a free 3-month health and wellness programme delivered by Harbour Sport. It aims to increase physical activity and improve the nutrition of adults aged 18 years and over. Healthy Lifestyle Advisors provide individuals with support and motivation to become independently active with the ultimate goal to improve health.
What does the service include?
- Face to Face healthy lifestyle consultations provided at Auckland Council Leisure Centres.
- West Wave Pool and Leisure Centre, Henderson
- Glenfield Pool and Leisure Centre
- Stanmore Bay Pool and Leisure Centre
- Albany Pool and Leisure Centre
- Discounted rates at a range of Auckland Council Leisure Centre’s as well as Green Prescription specific classes
- Goal setting, wellbeing and nutrition advice
- Nutrition support
- Facebook support and events
- Monthly follow-ups to support and motivate people to be independently active, eat healthily and live a
healthier lifestyle
How do I join the Green Prescription Programme?
On recommendation of your Health Professional, they will refer you to the Green Prescription programme using their internal referral system. They can also use the password protected Health Professional Referral Form.
If you are interested in this service, please see your Health Professional in the first instance.
Criteria
- Aged ≥ 18 years.
- Geographical boundaries: North Shore, Rodney, Waitakere and Hibiscus Coast.
- Currently inactive (less than 2.5 hours per week).
- Cleared for exercise by a GP or other health professional
- Are at risk of developing a medical condition due to a sedentary lifestyle.
- Ready to make changes to their lifestyle and consents to being referred.
Referrals can be by the general practice team (doctor or practice nurse), or any other health professional.
Objectives
The goals and objectives of Green Prescription are:
- Encouraging sedentary adults to become independently active.
- Supporting improvements in nutrition.
- Promoting healthier lifestyle choices for adults in Waitakere, North Shore and Rodney regions.
